We are looking for Subject Matter Experts (SMEs), network partners, trainees, and people with lived experience (PWLE). Collaborators will work with AUCD on a national project focused on disability-inclusive chronic disease management.
Through this project, we hope to improve chronic disease care for people with disabilities (PWD) through accessible education tools, a new Community of Practice (CoP), and lasting partnerships across the AUCD network.
As a collaborator, you will be part of either a development team or an advisory team. Team Opportunities: What is the difference between the Development and Advisory teams? 1. Advisory Team: Members will provide input and lived experience perspectives in several ways: Participate in structured listening sessions or focus groups Review draft materials to ensure accessibility and relevance Offer input on sustainability strategies and future directions
Ideal participants: Individuals with lived experience of disability and/or chronic disease, caregivers, and leaders from partner organizations.
Estimated commitment: ~ 4 hours per month from January - August 2026 (8 months)
2. Development Team: Members will be actively involved in several project objectives: Reviewing and co-developing accessible resources (fact sheets, briefs, and toolkits) Participating in pilot testing with AUCD network programs Serving as contributors or facilitators within the Community of Practice (CoP) Supporting partnership and sustainability activities
Ideal participants: SMEs in disability and public health, network partners, trainees, or individuals with lived experience.
Estimated commitment: ~ 10 hours per month from January – August 2026 (8 months) | | Project Objectives and Ways to Get Involved:
1.Creation of Resources for Knowledge Development on Disability and Chronic Health Conditions Review and contribute to accessible briefs, fact sheets, and toolkits on chronic disease management and disability inclusion Participate in pilot testing of educational resources with AUCD network programs Provide feedback and lived experience perspectives to ensure accessibility and relevance
2. Development of a Community of Practice (CoP) Help us develop a national Community of Practice focused on disability and chronic disease management Participate as a presenter, facilitator, or active member in CoP sessions Help shape the CoP’s priorities, peer exchange topics, and online resource hub
3. Promoting Partnership and Sustainability in the AUCD Network Contribute to collaborative meetings or listening sessions to align priorities and identify sustainability strategies Co-develop recommendations for long-term integration of disability-inclusive chronic disease management within AUCD’s public health portfolio Participate in national dissemination activities or planning meetings
